We are currently accepting most insurance plans. If you have any questions, please call (770) 971-5494. If we are not on your insurance plan, we are still more than happy to treat you and will give you the necessary paperwork to send to your insurance provider, though we cannot guarantee coverage. ___________________________________________________________         FINANCIAL POLICY Our fees apply to all patients, insured or uninsured.  We are open about our fees and will always provide you with an estimate.  Feel free to discuss any questions with us.  We accept cash, credit card, and personal checks. INSURANCE ·       Your insurance is a contract between you and your insurance company. ·       Insurance is your benefit. ·       As a courtesy, we will file your insurance. ·       Your deductible and co-payment portion is due on the day of service. ·       If you do not have your co-payment, we are not required to see you. ·       If your insurance is not paid within 60 days, your balance becomes your responsibility           and payment is expected at that time. ·       Failure to make payment in full or establish a payment plan with our office after 90 days           will result in your account being turned over to a collection agency. ·       You will be responsible for all collection costs and reasonable legal costs in addition           to the amount originally owed. ·       Returned checks will be charged a $35.00 fee.
